the survey results upon completing the survey.
For health care plans, the survey captures data on contribution strategies, contribution amounts, and plan
design features such as office visit copayments, deductibles,
out-of-pocket maximums, prescription drug benefits and more.
Salary continuation and income protection plans include plan
design features, eligibility requirements, maximum benefit
amounts, contributions, and more. The survey is structured to take into account
contribution levels that vary by employment class, tenure or
income level where appropriate.
To facilitate data
entry and analysis, the survey uses a multiple choice format where participants select from a menu of pre-defined benefit
ranges.
